Privacy Statement: Samson Centre For MS Privacy Statement

Section 1: What do we do with your information?

When you join the Samson Centre for MS, we collect the personal information you give us such as your name, address, phone number and email address. The Therapists will ask you for additional information e.g. name of your Doctor/Consultant, medical history, next of kin information etc.

Email updates: we may send you emails about fundraising events, updates at the Centre, dates for your diary etc.

Section 2: Consent to retain information about you

How do you get my consent?
There is an opportunity to say ‘no’ to receiving certain information e.g. fundraising updates sent out via Mailchimp email, general emails sent out from the Centre.

Please note that consent in this instance is in relation to the retention of personal data (not to be confused with consent which is asked for at the start of each therapy session).

How do I withdraw my consent?
You may withdraw your consent for us to contact you at any time, by contacting us at info@samsoncentre.org.uk or mailing us at: Samson Centre for MS, Guildford Waterside Centre, Riverside, Guildford, Surrey GU1 1LW.

Please note that if you withdraw your consent to share information with other medical professionals, this may affect the care we can offer you at the Centre.

Section 3: Disclosure

We may disclose your personal information if we are required by law to do so e.g. for public safety issues such as driving, use of illegal drugs or similar.

Your basic contact details are stored on the Northwood database. This is held on a secure server behind a firewall and accessed via an encrypted PC.

Section 5: Security

To protect your personal information, we take reasonable precautions and follow best practice to make sure it is not inappropriately lost, misused, accessed, disclosed, altered or destroyed. Physiotherapy documentation is kept in line with Chartered Society of Physiotherapy (CSP) and Health Care Professionals Council (HCPC) standards of practice.  Records are kept for 8 years and destroyed using a reputable shredding company. Gym and oxygen therapy records will also be kept for 8 years.

Section 6: Changes to this Privacy Policy

We reserve the right to modify this privacy policy at any time, so please review it frequently. We will email you if there are any changes to our privacy policy. Changes and clarifications will take effect immediately upon their posting on the website. If we make material changes to this policy, we will notify you here that it has been updated, so that you are aware of what information we collect, how we use it, and under what circumstances, if any, we use and/or disclose it.
